Virginia Family Chiropractic & Physical Medicine respects your privacy. This policy explains what information we collect when you visit our website or interact with our offices, how we use it, and the choices you have. A separate HIPAA Notice of Privacy Practices, included below, governs protected health information collected as part of treatment.

"Virginia Family Chiropractic," "we," "us," and "our" refer to Virginia Family Chiropractic & Physical Medicine, with offices in Alexandria, Falls Church, Manassas, and Woodbridge, Virginia. The primary practice address is 50 S Pickett St STE 114, Alexandria, VA 22304. Each of our four locations and corresponding phone numbers is listed on our contact page.

Information We Collect From Website Visitors

When you use this website, we may collect: (1) information you provide directly through contact forms, appointment requests, and the neuropathy self-assessment - such as your name, email address, phone number, location of interest, and any details you choose to share about your condition; (2) automatically collected technical data - such as IP address, browser type, device type, pages viewed, referring URL, and time on site; and (3) cookies and similar technologies as described below.

How We Use This Information

We use website information to respond to inquiries and appointment requests, to confirm and follow up on visits, to operate and improve our website, to measure the effectiveness of our advertising, to detect and prevent fraud and abuse, and to comply with our legal obligations.

Cookies, Analytics, and Advertising

We use Google Tag Manager to load measurement and advertising tags on our website, including Google Analytics 4 and Google Ads conversion tracking. These tools use cookies and similar technologies to understand site traffic, measure ad performance, and report when a visit leads to a contact-form submission, a neuropathy quiz submission, or other conversion event. The data collected through these tags may include your IP address, device and browser characteristics, pages viewed, and interactions on the site. We share information only with service providers that help us operate our website, communications, scheduling, and advertising - including hosting providers, email delivery providers, and the analytics and advertising platforms named above - and only as needed to perform their services. We may also disclose information if required by law, court order, or to protect the rights, safety, or property of patients, staff, or the public.

Data Retention

We retain website contact and form submissions for as long as reasonably needed to respond to your inquiry, schedule and document care, satisfy our legal and recordkeeping obligations, and resolve disputes. Patient medical records are retained under applicable Virginia and federal recordkeeping laws.

Your Choices and Rights

You can opt out of marketing emails using the unsubscribe link in any message. You may request access to, correction of, or deletion of personal information we hold about you by contacting us at the address below. Some information - including treatment records governed by HIPAA and Virginia medical-records law - must be retained even after a deletion request.

Children's Privacy

This website is intended for a general audience and is not directed to children under 13. We do not knowingly collect personal information from children under 13 through this website. Pediatric chiropractic care, when provided in our offices, is arranged with a parent or legal guardian.

Security

We use reasonable administrative, technical, and physical safeguards designed to protect the information we collect. No method of transmission over the internet is fully secure, and we cannot guarantee absolute security of information transmitted through our website.

HIPAA Notice of Privacy Practices

This Notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Pledge Regarding Health Information

Virginia Family Chiropractic & Physical Medicine is committed to protecting the privacy of your protected health information ("PHI"). We are required by the Health Insurance Portability and Accountability Act of 1996 ("HIPAA") and applicable Virginia law to: maintain the privacy of your PHI; provide you with this Notice of our legal duties and privacy practices regarding your PHI; and follow the terms of the Notice currently in effect.

How We May Use and Disclose Your Health Information

For Treatment. We may use and disclose your PHI to provide, coordinate, or manage your chiropractic and physical-medicine care, including sharing information among our chiropractors, nurse practitioner, supervising physician, and rehabilitation staff across our four offices, and with outside providers involved in your care (for example, primary-care physicians, imaging centers, or specialists you are referred to).

For Payment. We may use and disclose PHI to bill and collect payment from you, your insurance carrier, an auto- or workers'-compensation carrier, or a responsible third party - including verifying coverage, obtaining prior authorization, and providing claim documentation.

For Health Care Operations. We may use and disclose PHI for activities necessary to operate the practice, including quality assessment, provider review, training, licensing, accreditation, business planning, and administrative functions.

Appointment Reminders and Health-Related Communications. We may contact you to remind you of appointments, follow up after care, or inform you about treatment alternatives or other health-related benefits and services.

As Required or Permitted by Law. We may use or disclose PHI without your authorization when required by federal, state, or local law - including for public-health activities, reporting suspected abuse or neglect, health-oversight activities, judicial and administrative proceedings, law-enforcement purposes, organ donation, research under approved protocols, to avert a serious threat to health or safety, for specialized government functions, and for workers' compensation claims.

Uses and Disclosures That Require Your Written Authorization. Most uses and disclosures of psychotherapy notes, uses and disclosures of PHI for marketing purposes, and any sale of PHI require your written authorization. You may revoke a prior authorization in writing at any time, except to the extent we have already relied on it.

Your Rights Regarding Your Health Information

You have the right to: (1) request a copy of, or inspect, your medical and billing records, subject to limited exceptions; (2) request that we amend records you believe are incorrect or incomplete; (3) receive an accounting of certain disclosures we have made of your PHI; (4) request restrictions on how we use or disclose your PHI for treatment, payment, or operations - and to require us to restrict disclosures to a health plan for items or services you have paid for out of pocket in full; (5) request confidential communications at an alternative address or by alternative means; (6) receive a paper copy of this Notice on request, even if you have agreed to receive it electronically; and (7) be notified following a breach of your unsecured PHI.

Complaints

If you believe your privacy rights have been violated, you may file a written complaint with our Privacy Officer or with the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.

Changes to This Notice

We reserve the right to change this Notice and to make the revised Notice effective for PHI we already have about you as well as any information we receive in the future. The current Notice will be posted in each of our offices and on this page, with the effective date shown at the top.
